Dear Faiza,
-- Internee Evaluation Form, you are suppose to download from LMS, from your internship download section.
Take its printout and give it to your bank supervisor. Your bank will fill it and send through mail to the course instructor.
Just make sure that the dates and names of the Dept your bank supervisor put in the form has to be the same what u will put in your internship report.
Check your internship calendar for last date of submission of this form.Maybe its 29th June.
On evaluation form u will find a postal address, ask your Bank to send this form via mail.
